Make a Referral
We can offer support if you:
- Are a pregnant teenager or young mum
- Are aged 19 years or under
- Are in agreement with the referral
- Live within the Norwich City Council boundaries or in the following wards:
- Old Costessey
- New Costessey
- Cringleford
- Stoke Holy Cross
- Poringland
You can refer yourself or ask your midwife, health visitor, social worker or other person offering you support to refer you.

What happens next?
Once we receive your referral form we will contact you to arrange for an initial visit to you at home. This is usually a joint visit with the person who referred you, or a visit by two Embrace project workers.
At the joint visit we will talk to you about your situation and the help that Embrace can offer. This will help us to decide whether we can help you and which worker will be supporting you. You may not be supported by the project worker who does the initial visit.
Following this visit you will be sent a letter informing you who will be supporting you and how often they will visit. Shortly after this the project worker will contact you to arrange the next visit.
